Before the Procedure: Examinations

MRI (mandatory)

MRI of the problem area - no older than 3 months. If you have old images, bring them for comparison. MRI on disc (DICOM format) is preferable to film. You can have it done at your clinic and send it to us in advance for evaluation.

Blood Tests

Complete blood count (CBC). Coagulation panel. Tests are valid for 30 days. For chronic conditions, additional tests may be required.

Preliminary Consultation

Can be conducted online (video consultation) or in the clinic. Prof. Babayan will evaluate the MRI, determine indications, and create a treatment plan. Consultation is free if the procedure is subsequently performed.

Discontinue Medications

NSAIDs - 7 days prior

Ibuprofen, diclofenac, naproxen, meloxicam, celecoxib. NSAIDs suppress the inflammatory response necessary for activation of growth factors and stem cells. Alternative: paracetamol if needed.

Aspirin - 5 days prior

Aspirin irreversibly inhibits platelets - the main source of growth factors for CGF. If prescribed by a cardiologist, discontinuation only after consultation.

Corticosteroids - 2-4 weeks prior

Systemic (prednisolone) and local injections (dipropane, kenalog). Corticosteroids suppress regeneration and damage tissues. Last cortisone injection into the joint must be at least 4 weeks prior.

Anticoagulants

Warfarin, Xarelto, Eliquis, Pradaxa - discontinuation in consultation with treating physician and considering indications. Clexane (heparin) - discontinue 12 hours prior. Coagulation panel control is mandatory.

May Continue

Paracetamol, muscle relaxants, vitamins, blood pressure medications, diabetes drugs, thyroid medications, and other chronic medications (except those listed above).

One Week Before the Procedure

Nutrition

Adequate water: 2 liters per day (good hydration improves blood quality for CGF). Protein: meat, fish, eggs, cottage cheese (building material for regeneration). Vitamin C: citrus fruits, bell peppers, kiwi (cofactor for collagen synthesis).

Avoid

Alcohol - 48 hours prior (impairs platelet and stem cell quality). Smoking - minimize (nicotine constricts vessels, impairs microcirculation). Intense workouts - 48 hours prior.

Day of the Procedure

What to Bring

MRI images (disc or film). Test results. Passport. Comfortable clothing (access to procedure area). Companion - desirable but not mandatory.

Before the Procedure

Light breakfast (not on an empty stomach). Do not apply creams or lotions to the procedure area. Arrive 15-20 minutes before the scheduled time.

Procedure

Duration: 30-60 minutes (depending on area and volume). Outpatient - go home the same day. Local anesthesia - conscious, no general anesthesia. Post-procedure observation in clinic: 15-30 minutes.

What tests are needed before MIBRAR®?

MRI of the affected area (no older than 3 months), complete blood count, coagulation profile. For concomitant conditions - additional tests as agreed upon.

Which medications need to be discontinued?

NSAIDs - 7 days before, aspirin - 5 days before, corticosteroids - 2-4 weeks before, anticoagulants - as agreed upon. Acetaminophen is permitted.

Is a special diet required?

No special diet is necessary. Recommended: 2 liters of water daily, protein, vitamin C. Avoid alcohol for 48 hours.

Can I come alone?

Yes. The procedure is performed under local anesthesia, and you can move independently afterward. However, a companion is desirable for comfort, especially for procedures on the spine or hip joint.
