What is spondylolisthesis?

Spondylolisthesis is the slippage of one vertebra relative to another. It most commonly slips forward (anterolisthesis), less often backward (retrolisthesis). This leads to instability of the spinal segment, narrowing of the spinal canal, and compression of neural structures.

Spondylolisthesis occurs in 5-7% of the population, most frequently at the L4-L5 and L5-S1 levels. In athletes (gymnasts, weightlifters), the prevalence reaches 15-40%.

Degrees of slippage (Meyerding):

  • Grade I — slippage up to 25% of the vertebral body width
  • Grade II — slippage 25-50%
  • Grade III — slippage 50-75%
  • Grade IV — slippage more than 75%
  • Spondyloptosis — complete slippage

Types of spondylolisthesis:

  • Isthmus — pars defect (spondylolysis), most common
  • Degenerative — due to facet joint osteoarthritis, more common in the elderly
  • Dysplastic — congenital anomaly
  • Traumatic — after pars fracture

Frequently Asked Questions about Spondylolisthesis Treatment

At what degree of spondylolisthesis does MIBRAR® help?
MIBRAR® is most effective for grade I-II spondylolisthesis (displacement up to 50%). At grade III, significant symptom improvement and stabilization are possible. During consultation, Prof. Babayan will evaluate MRI images and functional X-rays to determine the optimal strategy.
Can MIBRAR® reposition the vertebra?
MIBRAR® does not mechanically reposition the vertebra but creates conditions for biological stabilization: strengthening ligaments, disc regeneration, reduction of facet joint inflammation. This stops progression of displacement and significantly reduces pain. In some cases, partial correction of vertebral position is observed.
What are the drawbacks of spinal fusion?
Spinal fusion permanently blocks mobility of the fixed segment. This leads to compensatory overload of adjacent segments and their accelerated degeneration — "adjacent segment disease" (Adjacent Segment Disease). Within 5-10 years, up to 30% of patients require reoperation. Other risks include: non-union, implant breakage, infection. MIBRAR® preserves natural mobility and does not overload adjacent segments.
Can spondylolisthesis be treated in athletes?
Yes, MIBRAR® is particularly relevant for athletes, where isthmic spondylolisthesis is common (up to 40% in gymnasts). Biological stabilization allows preserving full spine mobility and returning to sports activity, which is impossible after spinal fusion.
Is rehabilitation needed after MIBRAR® for spondylolisthesis?
No special rehabilitation is required. The patient goes home the same day. Moderate activity and core muscle strengthening (light exercises) are recommended. Follow-up functional X-rays and MRI at 8-16 weeks assess the degree of stabilization.
